In early 2016 Year of Care Partnerships developed a Community of Practice to enable those sites who had completed training to share their experience of implementing care and support planning with each other. 

The Community of Practice enables training sites to engage with others, share experiences, ask questions, share success stories and ideas, and support one another.

We are developing the Community of Practice with support from a virtual steering group to ensure it continues to meet it's member needs and provide an opportunity to share and learn.
