Webinar - PCSP in a post-COVID era

In June 2021 Year of Care Partnerships® delivered a webinar on the subject 'Year of Care personalised care and support planning for people with long-term conditions in a post-COVID era'. This webinar explored how personalised care and support planning can provide a clinical method and framework to provide high quality personalised care within a primary care setting for people who live with single and multiple long-term conditions. It supported attendees to develop an understanding of the core principles and practical application of personalised care and support planning. The webinar included how this can be set up, supported and practically applied within a primary care setting as well as looking at how the process can be modified to support remote delivery and provide a meaningful link to new roles within primary care.

Community of Practice - events and resources

In early 2016 Year of Care Partnerships developed a Community of Practice to enable those sites who had completed training to share their experience of implementing personalised care and support planning with each other. The Community of Practice enables training sites to engage with others, share experiences, ask questions, share success stories and ideas, and support one another.
