Closure of the national YOC programme - 16 Dec 2025
The pressures on funding and the uncertainties over the last 18 months about the future direction of the NHS has created a funding hiatus and as such we are going to need to close the programme at the end of November.
While this is naturally a significant moment for us, we are proud of what has been achieved. Over two decades, Year of Care has contributed to changing the way care is delivered for people with long term conditions in primary and secondary care, helping to establish more collaborative, person-centred approaches. These developments have been particularly successful thanks to the many individuals, organisations and communities who have worked with us and supported our efforts along the way.
Our materials and resources remain available in the document library.
